Billing Policy
This Billing Policy outlines the payment expectations, procedures, and responsibilities for all clients receiving services from Empowered Eating Nutrition and Wellness (“the Practice”), owned and operated by Tannaz Eslamparast. By booking and attending an appointment, clients agree to the terms outlined below.
Fees for Services
All services provided by the Practice are fee-for-service unless otherwise stated. Fees vary depending on the type and length of the session and will be communicated prior to booking.
Clients are responsible for full payment of all services received, including initial assessments, follow-up sessions, and any additional consultations.
Payment Methods
The Practice accepts the following forms of payment:
- Credit card (Visa, Mastercard, or other accepted providers)
- Debit (if applicable)
- Other approved electronic payment methods
A valid credit card must be kept securely on file for all clients.
Payment Timeline
Payment is due at the time of the appointment unless otherwise arranged in writing and be accepted by the Practice.
If payment is not completed at the time of the session, clients are provided a grace period of up to three (3) business days following the completion of the session to submit payment.
If payment is not received within this timeframe, the Practice reserves the right to process the outstanding balance using the credit card on file, in accordance with this policy.
Automatic Charging Authorization
By providing a credit card on file, clients authorize Empowered Eating Nutrition and Wellness to charge the card for:
- Outstanding balances not paid within three (3) business days after a completed session
- Late cancellation fees
- No-show fees
Clients will receive a receipt for all charges processed as per request.
Cancellation and No-Show Policy
The Practice requires at least 24 hours’ notice for appointment cancellations or rescheduling.
- Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee equal to the full session cost.
- Missed appointments (no-shows) will be charged the full session fee.
These fees will be charged to the credit card on file.
Insurance and Reimbursement
Clients are responsible for understanding their own insurance coverage.
The Practice may provide receipts for services rendered, which clients can submit to their insurance provider for reimbursement. However, payment to the Practice remains the responsibility of the client regardless of insurance outcomes.
Outstanding Balances
Clients with unpaid balances may be restricted from booking future appointments until the balance is settled.
The Practice reserves the right to pursue collection of unpaid fees through appropriate legal or administrative channels if necessary.
Refund Policy
All services are non-refundable once provided.
Refunds will not be issued for missed appointments, late cancellations, or completed sessions.
Changes to This Policy
Empowered Eating Nutrition and Wellness reserves the right to update or modify this Billing Policy at any time. Clients will be notified of any significant changes.
